The candidate will utilize the nursing process to provide for optimum quality and continuity of nursing care for patients in the behavioral health program. Duties include working directly with the provider in the clinic setting, managing incoming and outgoing phone calls and patient requests, and documenting in the electronic health record. Will also function as the case manager for an assigned group of behavioral health patients and their families to ensure the overall needs of the patient are met.
This role requires excellent communication and critical thinking skills; ability to utilize computers and software proficiently and a willingness to learn new skills. This role is a full-time position, day-time hours, Monday-Friday, and will work in all three clinics in addition to offsite visits with the providers.
Education required: current Illinois LPN/RN license; previous case management and/or behavioral health experience required.
