Description

Works within the Clay County Hospital Rural Health Clinics to provide assessments, individual care plans and counseling/therapy to Behavioral Medicine patients.  Actively participate as part of a multidisciplinary team to achieve specified outcomes for the individuals involved in treatment.

Master's Degree in Social Work or related field, LCSW certification required.

Candidate will demonstrate excellent customer service skills and be able to work independently.

Role is full time, Monday through Friday.
